There is a $25 charge for missing a scheduled appointment, or for canceling an existing appointment with less than 24 hrs notice. This fee applies only to well child visits and consultations.

Form Fees

There is a $10 fee for completion for all school, camp, work and sports forms. Please allow 1 week for forms to be completed.

Lateness

Out of respect for patients who come to their appointments on time, patients that are more than 20 minutes late for a well-child visit must reschedule. Those patients requiring a sick visit should expect to wait until the next available open appointment slot.

Consultation Fees

If you would like to meet one of our pediatricians prior to coming to the practice, please call the office to schedule a consultation. The fee for a consultation is $50.
